  • Michael Hauser, MPT, TPI Level 3 Certified Medical Professional - Physical Therapist / Owner

    Michael earned his Undergraduate Degree in Biology from Vanderbilt University and his Masters Degree in Physical Therapy from Nova Southeastern University. He’s worked with a wide variety of populations, ranging from people with neurological disorders to orthopaedic injuries.

    Over the last 25 years, his passion for golf led him to develop programs that treat golf related injuries and develop golf specific conditioning programs for both amateur and professionals. Michael is the only TPI (Titleist Performance Institute) Level 3 Certified Medical Professional, who is also a scratch golfer, on the Treasure Coast. Michael is also a founding board member of the Indian River Golf Foundation and enjoys helping golfers of all abilities, achieve their potential.

    Michael has completed continuing education courses in geriatric orthopaedics, foot biomechanics, manual therapy and myofascial release techniques to enhance his knowledge of the physical therapy profession. In his free time, Michael enjoys relaxing with his family and playing golf.

  • Shannon Hauser, MPT - Physical Therapist / Owner

    Shannon earned her Undergraduate Degree in Exercise Science from the University of Florida and her Masters Degree in Physical Therapy from Nova Southeastern University. While at the University of Florida, she specialized in Athletic Training which required more than 1500 hours of evaluating and treating sports related injuries. She worked in the University of Florida training room and with local high schools. She completed her athletic training internship with the Orlando Predators. She has spent most of her Physical Therapy career in orthopedics, mainly treating post-op patients.

    Shannon has enjoyed working with all ages including pediatrics. She spent two years developing a therapeutic horseback riding program for children with disabilities on the Navajo Reservation in Shiprock, NM. She has completed continuing education in positioning treatments for BPPV, manual therapy, cupping and geriatric therapeutic exercise. In her free time, Shannon enjoys spending her days at the beach, playing golf, riding horses, anything water related and spending time with her family.

  • Kaylie Smith, PT, DPT, OCS - Physical Therpist - Board Certified Orthopedic Clinical Specialist

    Kaylie attended Lake Erie College in Ohio, where she played collegiate softball and earned a Bachelor of Science degree. She then attended the University of St. Augustine, where she received her Doctorate in Physical Therapy. Kaylie is certified in LSVT BIG, enabling her to provide specialized care for individuals with Parkinson’s disease to improve movement amplitude, balance, and independence. She continues to expand her knowledge and specializes in orthopedics, vestibular therapy, and TMJ/TMD. Kaylie is passionate about helping people return to their sports and gain independence in daily activities. She enjoys going to the beach, reading mystery books, and spending time with her cats in her spare time.

  • Lacey Powell, PT, DPT - Physical Therapist

    Lacey attended Temple University where she was part of the Women's Soccer Team and earned a Bachelors of Science Degree in Kinesiology. She then went on to receive her Doctorate in Physical Therapy from Columbia University. She has worked with patients of various populations including neurological, orthopedic, and vestibular. She holds her LSVT certification for Parkinson's Disease and anticipates completing her dry needling certification in July 2025. In her spare time, Lacey enjoys golfing, traveling, going to the beach, and cooking.

  • Claire Petrulak, MSPT - Physical Therapist

    Claire is a licensed physical therapist with over 20 years experience specializing in outpatient orthopedics.  She earned her Masters of Science in Physical Therapy from Northeastern University in Boston, Massachusetts in 2025 and has spent her career in outpatient practices in the Boston area as well as here in Vero Beach. Claire's treatment philosophy is rooted in the belief that motion is essential to healing and she focuses on individualized, movement-based care that helps patients restore function and return to the activity they enjoy. 

    Claire incorporates principles from yoga and Pilates - both long-standing parts of her own active lifestyle into her treatment plans to enhance strength, mobility, balance and body awareness.  Originally from New York and raised in Vermont she developed an early passion for movement and rehabilitation though skiing.  In her spare time she enjoys spending time outdoors with her husband, a PGA golf professional, their two children and miniature Aussiedoodle

  • Lindsey Hunter, PTA - Physical Therapist Assistant

    Lindsey is a graduate of Indian River State College, where she earned her Associate of Science degree in Physical Therapy Assistant. She is certified in LSVT BIG, allowing her to provide specialized care for individuals with Parkinson’s disease, helping them improve movement amplitude, strength, and overall functionality. Lindsey enjoys working in the outpatient setting, where she takes pride in helping patients achieve their goals. In her spare time, she loves spending time with her daughters, working out, and traveling.

  • Madison Sweeney, PTA - Physical Therapist Assistant

    Maddie is a recent graduate of Indian River State College, where she earned both her AA in Health Science and AS degree as a Physical Therapist Assistant.

    In her free time, she enjoys staying active, spending quality time with friends and family, watching basketball, and exploring new places through travel. Maddie is also deeply involved in her church community, where she finds purpose and fulfillment through service and fellowship.

  • Melanie Moore - Receptionist / Scheduling

    Melanie has a background in home health and is a native of Vero Beach. At Back in Form, she especially enjoys meeting and getting to know patients, and then helping solve problems.
